CLIENT ADMISSION ​CRITERIA

• 18 years of age or older
  
• Able to communicate verbally
  
• Ambulatory
  
• Performs basic self-care
  
• Does not wander or run away
  
• Responds to staff instructions
  
• Basically cooperative
  
• Willing to participate in activities both indoor and outdoor
  
• Does not have a health condition that needs daily medical assistance and supervision and is not beyond the ability or training of non-medical staff

ENROLLMENT Process

1. ​​Carefully review our admission criteria and enrollment guide.
2. Contact our Admissions Team: Phone: 281-257-1004 or submit the form below.
3. Home Place will reach out to schedule a family tour.
4. Submit an application, along with a $50 non-refundable application fee (Tour and observation week are scheduled based on availability. Please arrange your schedule so that we are able to meet both you and the potential client.)
   1. ​If selected for an observation week, there will be a fee of $225.00 due before observation week begins. 
5. After your observation week, you and your client will have a brief consultation with staff about possible enrollment.  We look forward to meeting and providing you with resources that best fit your needs!
